Gov. Cox and the One Utah Health Collaborative Organizing Committee are inviting businesses and organizations throughout the state of Utah to participate in the One Utah Health Collaborative by pledging to support high-level state goals and principles developed through a community-oriented process which will guide the work of the non-profit Collaborative.

Organization representatives are asked to pledge their support online through this form. They are also welcome to join Gov. Cox at the Capitol to sign the pledge in-person on Sept. 6.
